Medical Advocacy Services:

Medical History Document Organization

Service: $150

(additional costs depending on complexity)

$150 base rate includes

25 pages of medical documentation for review and assist with organization.
Consultation and communication by patient portal powered by CharmHealth EHR and Mail/FormHIppo

Additional $100

for every 25 additional pages of medical documentation to review and organize.

Document Finding Fee: $250

To request consultant to assist with obtaining records from medical systems or providers. This fee is not required if you are able to source and submit your own medical records for review (preferred method).

The advocacy service is a one-time consultation and does not create an enduring therapeutic doctor-patient relationship. SESAME CARE PLATFORM

Traditional virtual healthcare visit appointments with a board-certified Family Physician
Acute care weekend appointments to address urgent care concerns such as a sinus infection, UTI, yeast infections from the comfort of your home. We provide work excuses for medical illnesses.
Ongoing follow-up care for refills and medication management are also designed for patients without a primary care home or do not have health insurance.
Medical Weight Loss Treatment - Explore options and access to cutting-edge FDA-approved weight loss medications to achieve and maintained desired health goals.
